नमस्कार मी डॉक्टर सौरभ पटवर्धन नंदादीप आय हॉस्पिटल याचा मी मेडिकल डायरेक्टर म्हणून काम करतो या व्हिडिओमध्ये मी तुम्हाला थोडक्यात माहिती देणार आहे ते डायबिटीक रेटनोपॅथी आणि त्याविषयी तुम्हाला जी ट्रीटमेंट सांगितलेली आहे म्हणजे तुम्हाला एक डोळ्यामध्ये इंजेक्शन घ्यायला सांगितलंय आणि त्यानंतर पडद्याचे लेझर सांगितलंय त्याविषयी ज्यांना डायबिटीक रेटिनोपॅथीचा आजार असतो त्यामध्ये दोन प्रकारचे आजार पडद्यामध्ये असू शकतात एक भाग असतो पडद्याला सूज येण्याचा जो आम्ही ओसीटी या मशीन द्वारे चेक करतो आणि त्याच्यामध्ये आपल्याला सूज दिसते आणि दुसरा प्रकार आहे पडद्यामध्ये जाळ्या तयार होण्याचा जे आम्ही डोळ्याचे पडद्याचे फोटो ज्याला आम्ही फंडस फोटो म्हणतो ते बघून आम्ही त्याच्याविषयीचा डायग्नोसिस करतो आता तुम्हाला जो आजार झालेला आहे याच्यामध्ये तुम्हाला पडद्याला सूज तर आहेच पण त्याचबरोबर पडद्याचा जो बाहेरचा भाग आहे त्याच्यामध्ये जाळ्या तयार झाल्या आहेत किंवा तयार होण्याच्या स्थितीमध्ये आहेत त्याला आम्ही पीडीआर किंवा सिव्हिअर एनपीडीआर असं म्हणतो त्यामुळे आपल्याला दोन गोष्टी आता साध्य करायच्या आहेत एक पडद्याची सूज कमी करायची आणि त्यासाठी तुम्हाला अँटी वेजे नावाचा आम्ही इंजेक्शन देऊ या इंजेक्शन नंतर सूज ही काही काळ कमी होते पडद्याची आणि याच कालावधीमध्ये आम्ही पडद्याचं लेझर ट्रीटमेंट करू यालाच पीआरपी किंवा पॅन रेटिनल फोटो कोगुलेशन असं म्हणतो आम्ही हे करून घेणे हे अत्यंत गरजेचे आहे याचं कारण असं आहे की जर तुमचं लेझर ट्रीटमेंट वेळेत झाली तर तुम्हाला अंधत्वाचा धोका जवळजवळ 95 टक्क्यांनी कमी होत असतो हीच जर तुम्ही वेळच्या वेळी जर लेझर ट्रीटमेंट करून नाही घेतली तर तुमचा अंधत्वाचा धोका हा वाढत असतो लक्षात घ्या तुमचा आत्ता कुठल्या स्थितीमध्ये पडद्याचा आजार आहे याविषयी तुम्हाला अर्थातच डॉक्टर सांगतील लेझर ट्रीटमेंट नंतर शक्यतो डोळ्याची दृष्टी ही वाढत नसते आपण ही ट्रीटमेंट अशासाठी करतो की डोळ्याची दृष्टी ही टिकावी आणि हा आजार वाढण्यापासून थांबावा काही वेळा लेझर ट्रीटमेंट झाल्यानंतर सुरुवातीच्या काही काळामध्ये थोडासा दुरकटपणा वाढण्याची शक्यता असते पण तो कालांतराने कमी होतो काही वेळा लेझर ट्रीटमेंट झाल्यानंतर पडद्यामध्ये पुन्हा सूज येऊ शकते जे आपल्याला इंजेक्शनच्या सहाय्याने नियंत्रणात आणता येईल येते इंजेक्शन किंवा लेझर ट्रीटमेंट ही पडद्याच्या आजाराच्या स्थितीप्रमाणे डॉक्टर ठरवत असतात काही वेळा या दोन्ही ट्रीटमेंट नंतर सुद्धा पुन्हा ट्रीटमेंटची गरज ही लागू शकते कारण डायबिटीक रेटनोपॅथी हा प्रोग्रेसिव्ह म्हणजे वाढत जाणारा आजार आहे त्याच्यामध्ये प्रत्येक रुग्णाचा ट्रीटमेंटला होणारा रिस्पॉन्स हा वेगवेगळा असू शकतो याच्यामध्ये अर्थात शारीरिक कारणं सुद्धा अवलंबून असतात जसं तुमची जर शुगर अनकंट्रोल असेल हिमोग्लोबिन लो असेल तुम्हाला हायपरटेन्शन किंवा बीपी असेल किडनीचा प्रॉब्लेम असेल किंवा इतर काही औषध तुम्हाला चालू असतील कोलेस्ट्रॉल वाढलेला असेल आणि तसेच आत्ता तुमच्या डायबिटीक रेटिनोपॅथीची काय स्टेज आहे त्यानुसार ह्या इंजेक्शन आणि लेझरचा रिस्पॉन्स अवलंबून असतो पण लक्षात घ्या ही सगळी जी ट्रीटमेंट आहेत हे तुमच्या आजार वाढण्यापासून थांबवण्यासाठी आहे आणि ती वेळेतच करून घेणे हे अत्यंत महत्त्वाचं आहे त्यामुळे तुम्हाला जर या इंजेक्शन आणि लेझरचा सल्ला दिला गेला असेल तर बिलकुल दिरंगाई करू नका आणि तुम्ही ही ट्रीटमेंट जरूर करून घ्या इंजेक्शन देण्यापूर्वी आपली शुगर कंट्रोल मध्ये असणं हे अत्यंत महत्त्वाचं असतं त्यामुळे तुम्हाला या इंजेक्शनच्या आधी प्री ऑपरेटिव्ह चेकअप सांगतील आणि त्यानंतरच आपल्याला इंजेक्शन देता येईल लेझर ट्रीटमेंट साठी शक्यतो आम्ही शुगर चेकअप फार महत्त्वाचा मानत नाही त्यामुळे काही रुग्णांमध्ये ज्यांच्यामध्ये जर शुगर अनकंट्रोल असेल आणि ती कंट्रोल येण्यामध्ये वेळ लागत असेल तर काही वेळा आम्ही लेझर ट्रीटमेंट लवकर चालू करतो अर्थात हे आपल्या शुगर कंट्रोल वर अवलंबून असतं आणि त्याचा निर्णय शेवटी डॉक्टर आपले रिपोर्ट बघून घेतात इंजेक्शन जर तुम्हाला सांगितलं असेल तर त्याविषयी तुमच्या फिजिशियन किंवा ज्यांच्याकडे तुमची ट्रीटमेंट चालू आहे डायबिटीसची त्यांच्याविषयी त्यांच्याशी तुम्ही याविषयी बोला आणि त्यांना याचं महत्त्व आम्ही अर्थातच लेटर द्वारे लिहून देऊ जेणेकरून ते तुमची शुगर कंट्रोल करण्यामध्ये तुम्हाला मदत करतील धन्यवाद
